What Youll Do

• Provide skilled nursing visits under the direction of your RN partner and the plan of care
• Perform wound care medication management vital signs and disease management
• Teach patients and families how to manage their health at home
• Report changes in condition to your RN partner and the care team
• Document accurately and on time

What Youll Need

• Active Oklahoma LPN license
• Valid drivers license and a clean driving record
• Home health experience preferred but if you have solid clinical skills and a heart for patients well teach you the rest
