Job Description
Title: CNA - Rehabilitation

Schedule:
- Shift: 12H Night
- Days: Monday to Sunday

Assignment Details:
- Contract Length: 6 weeks
- Guaranteed Hours: Not specified

Requirements:
- Minimum of 2 years of experience required
- Prior travel required
- Long-term care experience required
- BLS through AHA required
- Background check (nationwide, county court searches, FACIS3, and sex offender) within 18 months of start date
- FACIS 3 and VCIC within 30 days of start

Compliance / Notes:
- Floating required
- All work history information must be completed to submit; include bed count, EMR, facility type, etc.
- Employment gaps over 30 days must be explained
- Covid and flu declinations accepted; proof or declinations must be in packet
- Reliable transportation is recommended; personal or rental vehicle advised
- Vehicles should be equipped with all-season or snow tires during November to April
