We're looking for a compassionate Registered Nurse Case Manager to join our Beaumont team. What You'll Do: Manage a caseload of hospice patients throughout the Beaumont area. Complete comprehensive nursing assessments. Develop and update individualized plans of care. Coordinate care with physicians and the interdisciplinary team. Educate patients and families on disease progression and symptom management. Document visits using our EMR system. Participate in IDT meetings. Qualifications: Texas RN License. One year of nursing experience. Hospice or Home Health experience preferred. Strong assessment and communication skills. Reliable transportation. Benefits: Competitive salary. UHC Medical.
