We are looking for a Home Health & Hospice RN Case Manager who values relationships, enjoys working independently, takes pride in providing excellent care, and wants to be part of a supportive team where their work truly matters. This position is primarily focused on home health patients, with occasional hospice visits and support as needed.

What You'll Do

As an RN Case Manager, you will primarily provide skilled nursing care and care coordination for home health patients, while occasionally supporting hospice patients and families as needed.

Your responsibilities will include:

• Perform comprehensive nursing assessments and ongoing patient evaluations.
  
  
• Provide skilled nursing care, including wound care, catheter care, venipuncture, medication management, and patient/family education.
  
  
• Develop individualized, patient-centered goals and plans of care in collaboration with patients, families, physicians, and other members of the care team.
  
  
• Coordinate care with physicians and other healthcare providers.
  
  
• Monitor patient progress and adjust the plan of care as needed.
  
  
• Educate patients and families about medications, disease processes, treatment plans, and available resources.
  
  
• Supervise and direct patient care provided by other members of the care team.
  
  
• Maintain accurate and timely clinical documentation and ensure compliance with applicable regulations. Our EMR is Axxess.
  
  
• Advocate for your patients and help them remain as independent and comfortable as possible in their own homes.

Qualifications

• Current Utah Registered Nurse (RN) license .
  
  
• Current Utah driver's license and reliable transportation with required automobile insurance.
  
  
• Current CPR certification.
  
  
• Ability to pass a required background check.
  
  
• At least one year of nursing experience is preferred.
  
  
  
• Previous home health or hospice experience is a plus, but not required .
  
  
• New graduate RNs with a strong interest in home health and hospice are encouraged to apply. Training and support are provided.

Compensation: $36-$45 per hour, based on experience

Typical Schedule: Monday–Friday, 8:30 a.m.–5:00 p.m., with some flexibility.
