Admissions Director - Hamden Rehabilitation and Healthcare Center
Job Summary:
As the Admissions Director at Hamden Rehabilitation and Healthcare Center, you will play a vital role in leading our admissions team to provide exceptional care and support to our residents and their families. With a focus on compassionate service and team-oriented culture , we are seeking a dedicated and experienced professional to join our dynamic team.
Responsibilities:
Lead the admissions process, ensuring seamless coordination with hospital discharge planners, physician offices, and other healthcare providers
Develop and maintain strong relationships with community partners, healthcare professionals, and other stakeholders to drive referrals and admissions
Collaborate with our healthcare team to provide personalized care and support to residents, addressing their unique needs and goals
Manage and maintain accurate records, reports, and databases to track admissions, referrals, and resident outcomes
Develop and implement effective strategies to increase admissions, reduce wait times, and improve quality of care
Participate in quality improvement initiatives, sharing best practices and driving continuous improvement across the organization
