Job Details Job Location: Aylett, VA 23009 Position Type: Full Time Essential Job Functions Perform tasks pertinent to patient age (infancy to geriatric) Maintain accurate inventory of medical supplies and assist in the selection of division equipment and supplies Perform all nursing procedures as directed by the clinician according to the guidelines in the Nursing Procedure Manual Assist in quality assurance and control Support a team-based approach to patient-centered medical care Participate in the continuing educational process Assist in the efficient flow of patients Attend work as scheduled Perform related duties as assigned Triage Responsibilities and Process Triage means "sorting out"—a telephone triage nurse is a registered or licensed practical nurse who assesses patients over the phone. The role requires good clinical judgment, careful listening, and critical thinking and assessment skills. The nurse develops an assessment of the patient’s problem based on the conversation and determines whether self‑care advice, referral, appointment setting, or emergency action is required. Listening Skills Telephone triage nurses must be exceptionally skilled at listening to patients. They interpret subtle cues such as tone of voice, hesitations, or incomplete responses and ask appropriate questions. This skill is crucial when dealing with patients who speak limited English or are assisted by a translator. The Process of Care The nurse’s clinical judgment, knowledge, and experience guide the conversation and decision‑making. She may handle a wide spectrum of medical conditions, guiding patients to self‑care, primary care, scheduling appointments, or contact 911 for emergency assistance. Decision-Making Determining the level of care and appropriate action is a primary function. The nurse follows protocols to decide if a patient needs immediate care, can wait, or requires specialist referral. Patient education is provided when appropriate. Other Issues Triage is a high‑risk area, especially with obstetrics or conditions with potential for malpractice. The nurse must exercise skill and care in providing advice, ensuring the appropriate standard of care and documenting the advice and patient responses. Demeanor The nurse is expected to be considerate, patient, calm, and professional in appearance and communication, fostering patient comfort during discussions. Miscellaneous Responsibilities The nurse directs patients correctly, maintains accurate records of all calls, retrieves messages, creates documents, edits medical records, and reports to providers as needed. Follow‑up calls are made to monitor patient condition, with documentation of any changes or improvement.
