This role involves providing direct patient care in the home setting, utilizing all phases of the nursing process including assessment, diagnosis, planning, implementation, and evaluation. • Schedule: On-call 1 full week every month, involving evenings from 1700 to 0830 on weeknights and 24-hour coverage on weekends.
• Additional Information: Provides direct nursing care at the patient's residence, follows physician-ordered care plans, completes documentation timely, and participates in quality improvement activities. Builds partnerships with healthcare team members and ensures safety and quality of care.
