Pride Health is actively hiring an Occupational Therapist to join our client's Home Health Program in a growing care team serving patients


Job Description:
Position: Occupational Therapist - Home Health
Location: Portland, Oregon
Duration: 13+ Weeks Contract (Possibilities of extension)
Local Pay: $55.00 - $57.00/hour (W-2) Plus Mileage reimbursement
Travel Pay Package: Up to $2,536/week gross (includes weekly non-taxable stipend)* Plus Mileage reimbursement
Schedule: Monday-Friday | 8:00 AM-4:30 PM


On Call/ Weekend: No
Mileage Reimbursement: Reimbursement Provided - (from first patient to last patient, excluding travel to and from home)

Job Responsibilities:

• Perform occupational therapy evaluations in patients' homes
• Develop individualized treatment plans based on patient needs and goals
• Implement therapeutic interventions to improve mobility, strength, coordination, and daily living skills
• Monitor patient progress and adjust treatment plans as needed
• Educate patients and caregivers on therapy techniques, safety practices, and adaptive equipment
• Collaborate with physicians, nurses, and interdisciplinary care teams to ensure quality patient care
• Maintain accurate and timely documentation in accordance with agency and regulatory standards

Qualifications:

• Current Occupational Therapist licensure
• Home health/Home care experience
• Current CPR/BLS certification
• Valid driver's license
• Reliable transportation
