Physical Therapist - Full Time

We’re looking for a Physical Therapist to take on a full-time or part-time role with a skilled nursing facility in Sauk City, WI. You will help residents improve mobility, function, and quality of life in a therapist-led environment. Apply today and a recruiter will reach out with the details.

Minimum Requirements:

• Doctoral, Master’s, or Bachelor’s degree in Physical Therapy from an accredited program approved by the APTA.
• Current Wisconsin Physical Therapist license.
• BLS certification from the American Heart Association or American Red Cross may be required.
• Experience working with adult and geriatric populations, preferably in a skilled nursing facility or long-term care setting.
• Clinical documentation experience. Experience with Net Health is a plus.
• Authorization to work in the United States.

Your Responsibilities:

• Provide physical therapy services to adult and geriatric residents.
• Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence.
• Support residents in achieving rehabilitation goals and milestones.
• Accurately document care and maintain compliance with facility standards.
• Collaborate with therapy and nursing teams to ensure patient-centered outcomes.
• Educate residents and families on safe techniques and strategies to maximize independence.
• Participate in team meetings and contribute to care planning.
