Responsibilities
- Administer prescribed medications and treatments according to physician orders and facility policy
- Monitor residents’ conditions, identify changes, and communicate findings to the care team
- Perform routine nursing assessments and document results accurately in the chart
- Assist residents with activities of daily living and personal care needs
- Maintain safety practices, infection control standards, and proper equipment use
- Collaborate with RNs, physicians, and other clinicians to develop and implement individualized care plans

Requirements
- Active Licensed Practical Nurse (LPN) license in good standing (New Hampshire) or eligibility to practice
- Graduation from an accredited practical nursing program
- Ability to perform duties in a long-term care setting and work 12-hour shifts
- Strong communication and teamwork skills
