About the Role:

The Licensed Practical Nurse (LPN) in the Acute Inpatient Rehabilitation Unit plays a critical role in delivering high-quality nursing care to patients recovering from serious injuries, surgeries, or illnesses. This position focuses on supporting patients through their rehabilitation process by monitoring vital signs, administering medications, and assisting with therapeutic activities under the supervision of registered nurses and physicians. The LPN will collaborate closely with an interdisciplinary team to ensure individualized care plans are effectively implemented, promoting optimal patient outcomes and safety. This role requires a compassionate and detail-oriented approach to manage the complex needs of patients in an acute care setting. Ultimately, the LPN contributes to enhancing patients' functional independence and quality of life during their rehabilitation journey.

Minimum Qualifications:

• Current and valid Licensed Practical Nurse (LPN) license in the state of Ohio.
• High school diploma or equivalent required; completion of an accredited practical nursing program.
• Basic Life Support (BLS) certification.
• Demonstrated experience or clinical exposure in an acute care or rehabilitation setting preferred.
• Ability to pass background checks and meet health screening requirements.

Preferred Qualifications:

• Experience working in an inpatient rehabilitation or acute care hospital environment.
• Familiarity with electronic health record (EHR) systems and documentation standards.
• Strong interpersonal and communication skills to effectively collaborate with multidisciplinary teams.
• Continuing education or training related to rehabilitation nursing or patient care best practices.

Responsibilities:

• Provide direct nursing care to patients in the acute inpatient rehabilitation unit, including medication administration, wound care, and monitoring of vital signs.
• Assist in the development and implementation of individualized care plans in collaboration with registered nurses, therapists, and physicians.
• Observe and document patient responses to treatments and report any changes in condition promptly to the healthcare team.
• Support patients with activities of daily living and rehabilitation exercises as directed by the care plan.
• Maintain accurate and timely patient records in compliance with healthcare regulations and facility policies.
• Ensure a safe and clean environment by adhering to infection control protocols and safety standards.
• Communicate effectively with patients, families, and interdisciplinary team members to provide education and emotional support.

Skills:

The LPN will utilize clinical nursing skills daily to assess patient conditions, administer medications safely, and perform treatments as prescribed. Strong observational skills are essential to detect subtle changes in patient status and communicate these promptly to the healthcare team. Effective communication skills enable the LPN to educate patients and families about care plans and rehabilitation goals, fostering a supportive environment. Organizational skills are critical for maintaining accurate documentation and managing multiple patient care tasks efficiently. Additionally, proficiency with electronic health records enhances the ability to document care and coordinate with interdisciplinary team members seamlessly.
