Physical Therapist – Skilled Nursing Facility | Menlo Park, CAWe are partnering with a well-established 160-bed post-acute and skilled nursing facility in Menlo Park, CA to find an experienced, compassionate, and professional Physical Therapist (PT) to join their growing rehabilitation team. This is a permanent, full-time opportunity with an ASAP start — ideal for a clinician who thrives in a collaborative SNF environment.Position Details:Setting: Skilled Nursing Facility / Post-Acute Rehab (160-bed)Patient Population: GeriatricSchedule: Full-time | PermanentHours: Monday – Friday, standard business hoursLocation: Menlo Park, CAPay Rate: $65/hourStart Date: ASAPAbout the Role:As a Physical Therapist, you will evaluate and treat a geriatric patient population recovering from surgery, illness, or injury in a post-acute setting. You will manage an average caseload of 10 patients per day, with a productivity expectation of 87–90%. The facility uses PCC and Net Health as their EMR systems.You will be part of a team of 5 therapists in a flexible, cooperative environment where collaboration and patient-centered care are at the heart of everything they do.Key Responsibilities:Evaluate and develop individualized physical therapy treatment plans for geriatric patientsDeliver skilled therapeutic interventions including therapeutic exercise, functional mobility training, and manual techniquesMaintain 87–90% productivity while delivering high-quality patient careAccurately document patient progress and care plans in PCC and Net HealthCollaborate with an interdisciplinary team including OTs, SLPs, nursing, and physiciansSupervise Physical Therapy Assistants (PTAs) as applicableParticipate in discharge planning and care coordinationRequirements:Current California PT license (or eligibility to obtain)Graduate of an accredited Physical Therapy program (DPT preferred)Minimum 1–2 years of SNF or post-acute experience requiredProficiency or willingness to learn PCC and Net Health EMRStrong compliance, professionalism, and communication skillsTeam-oriented, flexible, and patient-focused work styleWhy Apply Through Us?This role is being placed exclusively through Welltech Partners.
