Description

American Traveler is hiring an experienced RN for a Hospice/Palliative Care Case Manager position requiring HCHB EMR experience and a minimum of one year of recent hospice experience.

Details

• Home hospice setting serving a territory of up to 60 miles from the hub/office
• Geriatric end-of-life care patient population
• Average caseload of 5–7 patients per day, with a minimum of 30 points per week
• Monday through Friday, 8:00 AM – 5:00 PM (5x8-hour days)
• Back-up on-call responsibilities required; details to be discussed during interview
• HCHB (HomeCarHomeBase) EMR experience is required
• Local candidates are accepted at the travel rate

Requirements

• Active RN license (Standard or Compact) currently in hand
• Current BLS certification required
• Minimum 1 year of recent hospice nursing experience required
• HCHB EMR experience required
• Must be flexible with territory coverage within the assigned service area

Additional Information

• Provide case management and direct nursing care to hospice patients in a home-based setting
• Timely documentation and communication are essential responsibilities of this role
• iPad device provided; personal cell phone is not required
• Mileage reimbursed at the IRS standard rate
• Scrubs required in any color; jeans are not permitted
• First-time travelers will not be considered for this position
• Any time off requests must be submitted at the time of initial consideration
• Full 7-year work history with all gaps explained is required, along with 2 professional supervisory references from within the past 2 years
