Job Title: RN - Case Management - Travel

Location: Southern Jefferson and Bullitt County, IN

Pay: $2,546.40 weekly

Shifts: M-F 8am-5pm; On-call required on rotating weekends

Seven Healthcare are seeking a dedicated RN specializing in Case Management for a travel assignment in Southern Jefferson and Bullitt County. This role requires providing comprehensive assessments and care coordination for high-risk members, within an onsite capacity. Key Responsibilities of the RN - Case Management - Travel

Perform comprehensive assessment of high-risk members
Collaborate with primary care providers to implement and engage comprehensive care plans
Continuously reassess and address cost-efficient, appropriate levels of care
Facilitate non-medical support to ensure compliance with treatment plans
Coordinate care transitions as needed
Work with physicians, social workers, and claims professionals in care transitions
Engage specialty resources as required for optimal resolution
Maintain detailed records of clinical, functional, and fiscal outcomes
Identify opportunities for health promotion and illness prevention
Act as patient advocate by protecting privacy and confidentiality issues

Requirements of the RN - Case Management - Travel

Associates or Bachelor's Degree in Nursing or related field
Current, unrestricted state Registered Nurse license
Ability to analyze complex medical information and make rational decisions
RN Licenses in IN, compact license accepted
