Responsibilities

• Conduct comprehensive occupational therapy evaluations for patients receiving home-based care and identify functional goals.
• Develop and implement individualized treatment plans to improve independence in activities of daily living, mobility, and upper-extremity function.
• Deliver therapy sessions, therapeutic activities, and training on adaptive equipment and techniques tailored to the home environment.
• Educate patients and caregivers on home exercise programs, safety strategies, and therapy goals; coach families to support ongoing progress.
• Document patient progress in the electronic medical record, communicate changes to the care team, and participate in discharge planning and follow-up planning.

Requirements

• Active Occupational Therapist license in Pennsylvania (or eligible for licensure)
• Master's or Doctorate degree in Occupational Therapy from an accredited program
• Minimum 1 year of related experience in acute care or home health settings preferred
• Ability to work effectively in a home healthcare environment and travel to patient homes
• Strong documentation and communication skills; detail-oriented and team-driven
