Day‑to‑Day Responsibilities As a Home Health PT at Inspired Homecare, you’ll conduct comprehensive assessments, create personalized treatment plans, and deliver high‑quality care to individuals in the comfort of their homes. You’ll work closely with a diverse team of healthcare professionals to ensure seamless coordination of care, monitor patient progress, adjust treatment plans as needed, and provide ongoing support to help patients achieve their goals. Qualifications A degree in Physical Therapy from an accredited university/program. A current, unrestricted state license as a Physical Therapist in the state of practice. Demonstrated proficiency in clinical assessments, documentation, and compliance with policies and procedures. Strong organizational and communication skills. Experience in Home Health Therapy and completion of OASIS documentation is preferred.
