Key Responsibilities

• Provide physical therapy services to patients in a skilled nursing facility (SNF) setting
• Evaluate, plan, and implement individualized patient treatment programs
• Document patient progress and maintain accurate clinical records
• Float between assigned worksites as needed during scheduled shifts
• Collaborate with interdisciplinary care teams to support patient recovery and functional goals

Required Qualifications

• Active California Physical Therapist license
• Minimum 1 year of experience in a skilled nursing facility (SNF) setting
• Ability and willingness to float to additional worksites as required
• Ability to work 8-hour day shifts, 5 days per week

Preferred Experience / Skills

• Experience with Casamba documentation software strongly preferred
• Familiarity with SNF clinical workflows and interdisciplinary team environments
