Nurse Practitioner - Greater Grand Rapids, MI - Sign On LTC Provider Care, PC is a medical group dedicated to serving patients in Skilled Nursing and Long-Term Care facility settings. Our goal is to improve the overall quality of care, patient outcomes, quality of life, refine facility processes, and enhance patient relationships. LTC provides Primary Care Physician (PCP) services to SNF-LTC facilities across Michigan. LTC is seeking a dedicated Nurse Practitioner to join our team. Job Structure This is an in-person position Providers will commute directly between home and their assigned facility After-Hours on-call is shared among all LTC Providers (currently a 13-week rotation) Standard Monday-Friday Employment Type Full-Time Requirements and Qualifications Experience in Family Medicine, Internal Medicine, or Geriatrics is strongly preferred. Candidates must hold a valid RN and NP license in Michigan. Candidates must hold a valid DEA license in the state of Michigan. Candidates must be board certified.
