Registered Nurse - Case Management - Hospital

Job Summary:

The Case Manager RN is responsible for care coordination and care transitions for acute care patients, collaborating closely with physicians, social workers, and the interdisciplinary team to ensure timely patient movement and optimal resource utilization. This role emphasizes discharge planning, utilization management, and seamless care transitions to improve patient outcomes.

Responsibilities:

• 
  Lead comprehensive patient assessments, including medical, psychosocial, and financial evaluations to determine care needs and risk levels.
  
  
• 
  Oversee and facilitate care transitions by reviewing plans, managing barriers, and coordinating post-acute care services.
  
  
• 
  Collaborate with physicians and healthcare team members to ensure accurate documentation supporting appropriate level of care and discharge planning.
  
  
• 
  Monitor length of stay, review hospital records, and expedite discharge planning for efficient patient throughput.
  
  
• 
  Communicate effectively with patients, families, and community resources to develop individualized care transition plans.
  
  
• 
  Ensure compliance with regulatory requirements, including CMS and Joint Commission standards, and document all activities thoroughly in the electronic health record.
  
  

Qualifications:

• 
  Current California RN license with active Nursys verification.
  
  
• 
  Minimum of 5 years of RN case management experience, specifically inpatient acute care.
  
  
• 
  Demonstrated discharge planning expertise, with a focus on hospital-based case management.
  
  
• 
  Valid BLS certification and compliance with all certification expiration requirements.
  
  
• 
  Strong collaborative, communication, and interdisciplinary teamwork skills.
  
  
• 
  Knowledge of regulatory standards related to transitions of care and CMS guidelines.
