Physical Therapist - Travel Contract

Minimum Requirements

Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
Active Michigan Physical Therapy license required to start the assignment.
BLS certification from AHA or ARC may be required.
Experience in skilled nursing facility settings preferred.
Productivity expectation: ability to meet approximately 30 points per week.
Familiarity with or willingness to use the EMR: Home Care Home Base preferred.
Mileage reimbursement follows IRS standard mileage rate; reliable vehicle required for travel between patient locations.
Weekend coverage: ability to work approximately one weekend per month, as needed (may not be necessary).
Home health experience is preferred but candidates new to the setting will be considered.

Clinician may be expected to travel within roughly a one-hour radius from Alpena to cover referrals.
