Job Description
Title: Physical Therapist

Schedule: 8-hour shifts

Assignment Details:
- Contract Length: 14 weeks
- Guaranteed Hours: 40

Requirements:
- Current State License as a Physical Therapist
- Proof of current CPR certification
- Current driver's license with reliable transportation and insurance
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting
- Previous home care experience (preferred)
- Bachelor of Science in Physical Therapy from an approved program

Roles & Responsibilities:
- Conduct clinical assessments to identify patient needs and determine care appropriateness.
- Document and complete a Plan of Care (POC), progress notes, and other clinical records on an ongoing basis, and revise as necessary based on ongoing assessment data.
- Plan and implement appropriate treatment for patients with neuromuscular, neurological, and orthopedic disorders.
- Instruct patients and families in safe transfer techniques and the use of therapeutic equipment.
- Provide patient and family education regarding therapeutic procedures based on individual needs.
- Coordinate with interdisciplinary teams and oversee the performance of care by other team members when applicable.

Compliance / Notes:
- Must meet mandatory continuing education requirements.
- Applies concepts of infection control and universal precautions in patient care.
- Must read, write, and speak fluent English.
