Position Summary

The Licensed Practical Nurse (LPN) is a graduate of an accredited nursing program and holds Louisiana state licensure as a Licensed Practical Nurse in good standing. The LPN has duties and responsibilities related to patient care in the ambulatory clinic environment. Nursing triage is part of a care coordination team supporting care processes alongside providers. Triage includes but is not limited to managing nurse-directed patient telephone calls and walk-in patients requesting health care services. The LPN utilizes clinical judgment within the LPN scope of practice to prioritize patient needs, identify urgent or emergent conditions, and ensure appropriate and timely disposition of care in collaboration with the provider and care team.

Key Duties and Responsibilities

• Delivers quality patient care.
• Maintains accurate, complete, and timely documentation.
• Assists with inventory and medication management.
• Maintains a safe and compliant clinic environment.
• Maintains examination rooms between patient appointments as needed. • Manages patient flow.
• Triages patient calls and walk-ins for urgent appointments.
• Applies standardized triage protocols and evidence-based guidelines to assess patient symptoms and determine appropriate level of care.
• Prioritizes patient concerns based on acuity, risk factors, and presenting symptoms.
• Provides clear guidance to patients regarding home care, urgent care, emergency services, or scheduled appointments based on assessment.
• Escalates complex, high-risk, or unclear cases to the provider.
• Documents triage interactions thoroughly, including assessment, disposition, and patient instructions.
• Promotes teamwork, good interpersonal relations, and communication.
• Participates in ongoing training, maintains clinical competency, licensure, and performs functions within the LPN's scope of practice.

POSITION REQUIREMENTS

Education
• The minimum education level required for the position is High School Diploma or GED, and completion of an accredited LPN training program, community college, and /or vocational school.

Work Experience
• Minimum of 3 years of experience working as a nurse in a clinic or hospital setting required. Other (licensure, certifications)
• Licensed Practical Nurse in good standing without restrictions required.
• Basic Life Support (BLS) certification required. ESSENTIAL PHYSICAL REQUIREMENTS
• Extended periods of sitting required.
• Able to lift up to 25lbs.
• Work is primarily office based.
• Some travel may be required, including but not limited to travel between clinic locations as needed.
