Seven Healthcare are seeking an experienced Physical Therapist for a contract assignment in Cullman, AL. This role involves providing comprehensive physical therapy treatments and evaluations within a supportive and professional work environment. - Perform physical therapy evaluation and treatment of patients based upon physician orders/referrals
- Complete comprehensive evaluations using appropriate evaluation and assessment tools
- Review patient records for diagnosis, history, precautions, and contraindications
- Establish measurable goals and develop patient treatment plans
- Reassess treatment plans and progress towards goals regularly
- Communicate patient needs and progress to the treatment team
- Consult with medical providers on patient evaluation, treatment, and discharge
- Recommend and assist in securing adaptive equipment for patients
- Maintain and submit evaluation, treatment, and billing documentation
- Supervise Physical Therapist Assistants and Aides in patient care
- Attend and participate in staff development programs and meetings
- AL PT License (Compact license accepted)
- Proficient in OASIS and HCHB
- Previous PT experience required
