In this pivotal role, you will provide clinical oversight for Nurse Practitioners (NPs) and Physician Assistants (PAs) serving patients in assisted living facilities and group homes. Your expertise in internal or family medicine, particularly in caring for geriatric and medically complex populations, will enhance patient care outcomes. You will conduct periodic oversight visits to review patient care plans and provide evidence-based clinical recommendations, all while working solely in an advisory capacity without direct patient management responsibilities. Board certification or eligibility in Internal Medicine or Family Medicine is essential, alongside substantial clinical experience in a primary care setting. Successful applicants will demonstrate comfort in reviewing and advising on care plans devised by Nurse Practitioners (NPs) and Physician Assistants (PAs), showcasing strong documentation skills and familiarity with Medicare standards.Preferred qualifications include experience in geriatric care, particularly within assisted living or group home environments, and knowledge of best practices in medication management and functional assessments. A background in mobile, home-based, or facility-based care models will further enhance your ability to collaborate effectively and support high-quality care delivery to our vulnerable populations.
