PRIMARY CARE PHYSICIAN

COMMUNITY OUTPATIENT CLINIC IN HYANNIS

HYANNIS, MA

Our organization is in search of a committed Primary Care Physician to join our team at the Hyannis, MA Outpatient Clinic, dedicated to serving the brave individuals who have defended our country. • Comprehensive Primary Care: Assess and manage a wide variety of acute and chronic health issues, perform physical examinations, order and analyze diagnostic tests, and create patient care plans.

• Collaborative Teamwork: Work alongside a Patient Aligned Care Team (PACT) that includes an RN care manager, LPN, clinical pharmacist, psychologists, and social workers to provide coordinated, holistic care.

• Conduct thorough health assessments, determine diagnoses and healthcare needs, and develop care plans based on effective therapeutic practices, consultations, patient education, and established guidelines.

• Demonstrate the ability to coordinate patient care through management techniques within the assigned Patient Aligned Care Team (PACT) and implement similar functionalities in PACTs.

Applicants must be U.S. citizens, hold an M.D. or D.O. degree, possess a full, active, and unrestricted medical license, and have at least one year of experience in outpatient Primary Care or Internal Medicine.
