The Network Resident Housing Liaison (RHL) is responsible for compliance activities and processes from initial audit to issue resolution to minimize the risk to InnovAge PACE/LIFE participants in contracted facilities./ppEssential Functions and Work Responsibilities/ppFunctional Category: Compliance, Risk Management, and Account Management Contracted Resident Housing Facilities/ppEstimated Percent of time Spent 70%/pp Assists with network expansion activities for InnovAge PACE/LIFE by participating in pre-vetting audits of new Resident housing provider contract requests./pp Proactively gathers information and monitors survey results for Resident housing providers to identify adverse results./pp Performs routine audits of contracted Resident care housing providers and prepares reports Director-Network Resident Housing Oversight and center leadership./pp Proactively works with PACE/LIFE leadership and Interdisciplinary team to ensure effective partnership and identify new partnership needs with contracted Resident housing providers./pp Facilitates resolution of care, quality or collaboration issues in contracted Resident housing providers and ensures special case monitoring and action planning to increase quality, compliance and mitigate risk./pp Recommends and assists with the implementation of corrective action with center leadership to improve quality of care and to increase collaboration and contractual compliance in housing providers./pp Monitors the effectiveness of corrective action and coordinates follow up through ongoing account management of the Resident housing provider and acts as the InnovAge first point of contact./ppFunctional Category: Compliance and Risk Management PACE/LIFE/ppEstimated Percent of time Spent 30%/pp Provides support to InnovAge PACE/LIFE Center leadership to ensure compliance with contractual Resident care housing agreement./pp Performs desk review of internal quality data and documentation, departments meeting minutes, and functions to ensure adherence to regulatory guidance./pp Prepares requested reports by collecting, analyzing, and summarizing relevant information obtained through Resident care housing facility investigations, and other compliance activities./pp Assists Director-Network Resident Housing Oversight with policy and procedure development./pp Provides the on-boarding and orientation of contracted Resident housing facilities in collaboration with the Center leadership and Network./pp Performs other related duties as assigned./ppstrong Travel Requirements /strong/ppEstimated Percent of Time Spent: 60%/pp Travel between local InnovAge Centers/pp Travel to targeted (network development) and contracted Resident Housing (Long-Term Care and Assisted Living) Facilities/pp Travel out of state as needed/ppstrong Qualifications /strong/ppTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily. An R.N/LPN or Clinical degree/license preferred, or clinical equivalent./ppLicensure, Certification, Registration or Designation/pp Requires valid state issued drivers license, personal transportation, safe driving record and auto insurance as required by law./ppstrong Work Experience and Qualifications /strong/pp A minimum of 3 years experience with current and relevant knowledge of applicable state and federal regulations for Long-Term Care and Assisted Living facilities./pp Registered Nursing License- Preferred./pp Current knowledge of P.A.C.E.
