Physical Therapist

Role: A compassionate and motivated Physical Therapist responsible for evaluating, developing, and implementing individualized treatment plans to help patients restore function, improve mobility, reduce pain, and maximize independence. Key Responsibilities Assess patients and create personalized treatment strategies. Deliver skilled physical therapy using evidence-based techniques. Monitor progress and adjust treatment plans accordingly. Educate patients and caregivers on exercises, safety, and assistive devices. Collaborate with physicians and multidisciplinary teams to enhance patient outcomes. Complete accurate clinical documentation and participate in discharge planning. Mentor assistants, students, and support staff, and engage in quality improvement and continuing education initiatives. Ensure compliance with HIPAA, accreditation standards, and organizational policies. Qualifications & Skills Graduate of an accredited Physical Therapy program (DPT preferred). Current or eligible Illinois Physical Therapist license. CPR/BLS certification or ability to obtain. One to two years of clinical experience preferred; new graduates encouraged to apply. Strong assessment, communication, and critical thinking skills. Ability to work effectively within a team and deliver compassionate, patient-centered care.
