Physical Therapist (PT) - Home Health

Buena Vista Home Health, Palliative Care & Hospice (BV) provides positive, life-changing services to individuals in Ventura & Los Angeles Counties. Job Summary Buena Vista is seeking a Physical Therapist (PT) to join our home health team. The PT is responsible for providing physical therapy services to patients according to a written physician’s plan of care. Duties & Responsibilities Assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care Evaluate home environment Assess for muscle strength, mobility, gait, ROM—potential for rehab Direct physical therapy treatment Instruct patients and families/caregivers in the use and care of therapeutic appliances Determine priority needs for physical therapy Report to physician patient’s reaction to treatment or changes in condition Initiate physical therapy program and instruct other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient Instruct them as to the goals of the physical therapy program for the patient by participating in case conferences Train patient in the use of prosthetic device Identify patient and family/caregiver needs for other home health services Prepare and submit clinical and progress summaries based on the attainment of goals Participate in discharge planning for patient Prepare and submit a clinical progress summary based on the attainment of goals Provide physical therapy consultation to home families/caregivers Job Requirements Active CA PT license Minimum of two (2) years of patient care experience; preferable in the home health setting Possess and maintain current CPR certification Licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order Pay Range $50.00 - $60.00 per hour About the Company Buena Vista Home Health is a subsidiary of the Pennant Group network with 300+ locations throughout the country.
