Job Details Job Location: 201 - JOL Home Health Kyle - Kyle, TX 78640 Position Type: Full Time Salary Range: $60,000.00 - $65,000.00 Salary/year Job Category: Nurse Essential Functions Assist in coordinating patient care services and staff assignments. Monitor patient condition and report changes to RN and physician. Support compliance with agency policies, procedures, and regulatory requirements. Document care coordination activities accurately and timely. Participate in interdisciplinary team meetings and case conferences. Provide patient and caregiver education as directed. Promote infection control and safety practices during patient care. Support quality improvement initiatives and staff training. Assist with staff competency assessments and evaluations. Respond to PRN emergency calls and participate in on-call rotation as assigned. Qualifications Education Graduate of an accredited vocational nursing program. Licensure Active, state license as a licensed vocational/practical nurse, or a multi-state license issued by a Nurse Licensure Compact (NLC) member state. Current driver’s license. Experience Minimum two years of clinical experience; one year of home health experience preferred. Management experience preferred. Skills Strong clinical skills and understanding of home health care standards. Effective communication and interpersonal skills. Ability to work independently and manage multiple priorities. Proficiency in documentation and use of EMR systems. Environmental and Working Conditions Works in patients’ homes in various conditions, possible exposure to blood, bodily fluids, and infectious diseases. Requires ability to travel locally and work a flexible schedule. May be exposed to unpleasant weather conditions. Physical and Mental Effort Requires prolonged standing and walking, ability to lift up to 50 pounds, and move patients.
