Registered Nurse (RN) Home Health

Pride Health is seeking a compassionate and experienced Registered Nurse (RN) to provide skilled home health services throughout Jackson, Clinton, Vicksburg, Bolton, Edwards, Raymond, Utica, MS, and surrounding areas. This role focuses on delivering high-quality patient care in the home setting, promoting independence, improving health outcomes, and providing education to patients and caregivers on disease management, medications, safety, and self-care strategies.

Schedule & Work Details

Location: Jackson, Clinton, Vicksburg, Bolton, Edwards, Raymond, Utica, MS, and surrounding areas

Patient Care Setting: Home Health

Schedule: Monday-Friday, 8:00 AM - 4:30 PM

Weekly Hour Guarantee: 40 Hours

Productivity Expectation: 30 Points

Pay Rate: $51-$53/hour

On-Call Requirement: 1-2 weekends per month

Mileage Reimbursement: Yes, reimbursed between patient visits (first patient to last patient)

Key Responsibilities

• Provide skilled nursing care to patients in their homes in accordance with physician orders and individualized plans of care.
• Perform comprehensive patient assessments and ongoing evaluations of patient conditions.
• Educate patients and caregivers on disease management, medications, treatment plans, safety precautions, and self-care techniques.
• Develop, implement, and revise patient-centered care plans designed to promote independence and improve outcomes.
• Monitor patient progress and communicate significant changes in condition to physicians and members of the interdisciplinary team.
• Complete accurate and timely clinical documentation in accordance with agency, Medicare, and regulatory requirements.
• Collaborate with physicians, therapists, case managers, caregivers, and other healthcare professionals to coordinate care.
• Participate in case conferences and contribute to care planning discussions.
• Maintain compliance with infection control standards, agency policies, and state and federal regulations.
• Participate in on-call rotations as assigned.

Required Qualifications

• Current, unrestricted Registered Nurse (RN) license.
• Associate Degree in Nursing or higher.
• Strong clinical assessment and patient care skills.
• Excellent communication and documentation abilities.
• Ability to travel throughout the assigned coverage area.

Preferred Qualifications

• Previous home health experience.
• Experience using Homecare Homebase (HCHB) and PointCare.
• Knowledge of Medicare home health regulations and documentation requirements.
• Ability to work independently and effectively manage a patient caseload.
• Strong organizational, critical-thinking, and time-management skills.
