Assignment Details:

• Contract Length: 13 Weeks

• Schedule: Monday - Friday, 8:00 AM - 5:00 PM (40 hours/week)

• Travel Area: Patient Service Area: Glynn, Camden, Charlton, Brantley, Wayne, Long, and Mcintosh Counties

• Productivity Units: 30 units

Responsibilities:

• Deliver skilled nursing care in patients' homes, following established care plans.

• Complete comprehensive assessments, including OASIS documentation.

• Administer medications, perform wound care, and provide patient/family education.

• Collaborate with interdisciplinary teams to coordinate patient care.

• Ensure timely and accurate documentation using Homecare Homebase (HCHB).

• Manage admissions, recertifications, and discharges.

• Participate in on-call rotation as required.

Qualifications:

• Required:

  • Active LVN/LPN license in Georgia (or compact state).

  • 1-2 years' experience as an RN in a clinical care setting.

  • 2+ years of home health experience.

  • Proficiency with HCHB software documentation.

  • Current CPR certification.

  • Reliable transportation, valid driver's license, and insurance.
