Job Description

This position requires experience with OASIS (Outcomes and Assessment Information Set) and involves a variety of nursing responsibilities. The successful candidate will utilize the nursing process to assess patient and caregiver needs, develop and communicate a realistic plan of care, and manage case loads effectively.

Key Responsibilities

• Assess patient and caregiver needs to provide quality care in accordance with evidence-based practice and regulatory standards.
• Develop and communicate realistic patient care plans, ensuring inclusion of patient and caregiver input.
• Serve as Case Manager for assigned patients, managing visit times in accordance with agency productivity standards.
• Complete OASIS documentation accurately and in accordance with agency policies and regulatory requirements.
• Implement medical plans of care and nursing interventions to ensure safety and quality of patient care.
• Stay updated on high-tech skills such as IV use and maintenance, wound VAC, and complex wound care.
• Collaborate with Licensed Practical Nurses (LPNs) to ensure adherence to care plans and delegate necessary tasks.
• Evaluate the effectiveness of the care plan, making adjustments based on patient responses and needs.
• Maintain timely and accurate documentation of nursing care provided, ensuring integration of information for ongoing quality of care.
• Follow agency procedures for documentation processes, including locking down visits and completing necessary notes.
• Educate patients and caregivers regarding medication side effects, treatment options, and post-discharge instructions.
• Identify interdisciplinary needs and communicate relevant information to other healthcare professionals in a timely manner.
• Accurately complete various agency and regulatory forms to facilitate patient care.
• Conduct visits efficiently, ensuring supervisory visits are distinct from routine visits as required.
• Assess patient needs for Home Care Aide (HCA) services, coordinating plans of care with patients and caregivers.
• Transmit incident reports related to patient safety in a timely manner.
• Coordinate safe patient discharges and transfers, ensuring all necessary paperwork is processed correctly.
• Facilitate communication with physicians, providers, and team members to support recommended care plans.
• Participate in monthly peer review audits, providing and accepting constructive feedback professionally.
• Schedule and facilitate case conferences with management and team members to enhance communication and care effectiveness.
• Advocate for patient care issues, ensuring that the type and quality of care meet the specific needs of each patient and family.

Qualifications

Candidates must possess the required qualifications, including relevant experience and up-to-date knowledge of nursing standards and practices. OASIS experience is essential.
