Position Summary

Providence Healthcare of Thomaston is seeking a compassionate and motivated Physical Therapist (PT) to join our rehabilitation team on a per diem basis. The Physical Therapist will evaluate residents, develop individualized treatment plans, and provide skilled therapeutic interventions designed to improve mobility, strength, balance, endurance, and overall functional independence.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans
• Establish functional goals and implement appropriate therapeutic interventions
• Provide skilled physical therapy services including therapeutic exercise, gait training, balance training, transfer training, mobility enhancement, and pain management techniques
• Assess resident progress and modify treatment plans as appropriate
• Supervise and direct Physical Therapist Assistants in accordance with state practice guidelines
• Complete evaluations, progress notes, recertifications, and discharge summaries accurately and timely
• Educate residents, families, and caregivers on treatment plans, exercises, safety techniques, and mobility strategies
• Collaborate with physicians, nursing staff, therapists, and interdisciplinary team members to support resident care plans
• Ensure compliance with all federal, state, and facility regulations and standards

Qualifications

• Current Georgia Physical Therapist license in good standing
• Graduate of an accredited Physical Therapy program
• Previous experience in skilled nursing, rehabilitation, or long-term care preferred
• Strong communication and interpersonal skills
• Ability to work independently and within a multidisciplinary team
• CPR certification preferred
