As a MSW Social Worker, you will:

We are seeking a compassionate and detail-oriented MSW Case Manager to join our Continuing Care team. This role focuses on delivering high-quality, patient-centered care coordination while supporting diverse populations, including Medical and Medicare members. The ideal candidate brings a strong clinical foundation, excellent communication skills, and the ability to think critically in a fast-paced, collaborative environment.

Although the role is primarily remote, candidates must be available to pick up equipment and attend one day of onsite training in Pasadena .

Key Responsibilities

• Conduct comprehensive case management activities using a holistic, patient-centered approach
• Develop and implement basic care plans tailored to individual member needs
• Coordinate care across multidisciplinary teams to ensure continuity and quality outcomes
• Identify and respond appropriately to urgent or emergent situations
• Engage with members through active listening, asking insightful questions, and adapting communication as needed
• Support members in navigating healthcare systems, including Medical/Medicare services
• Collaborate with internal teams and proactively seek guidance when needed
• Document interactions, care plans, and progress accurately and timely

Requirements

• Master of Social Work (MSW) required
• (Candidates without MSW may be considered if they hold a valid LCSW)
• Strong customer service and interpersonal skills
• Ability to think critically, prioritize, and adapt in a dynamic environment
• Excellent communication and listening skills
• Ability to work both independently and collaboratively within a team
• Basic knowledge of care planning and case management processes

The ideal candidate will have: A Master of Social Work (MSW) or LCSW, strong critical thinking and communication skills, a patient-centered approach to care, and experience supporting Medical/Medicare populations in a collaborative, fast-paced environment.

PAY RANGE AND BENEFITS:
Pay Range*: $27 - $32 per hour
*Pay range offered to a successful candidate will be based on several factors, including the candidate's education, work experience, work location, specific job duties, certifications, etc.
