Assesses the patient's skin condition and wound care needs upon admission and weekly; sets goals; recommends appropriate nursing actions to meet those goals. Keep other licensed clinical professionals informed on items related to wound care, ostomies, and incontinence. Monitor and track changes in the client/patient's conditions and needs, and keeps the Director of Nursing and any other appropriate personnel informed.Requirements:Current unrestricted Texas Registered Nurse (RN) licenseCurrent CPR/BLS Certification (in-person only) Negative TB skin test or chest x-ray within past yearMinimum one (1) year of documented experience required in a home health agency, primary care clinic or health facility, preferred.WOCN preferredCurrent and valid Texas Driver's LicenseCurrent and valid automobile insuranceMust be able to stand, walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client.
