Post Acute Care Opportunity
Post Acute Care opportunity. Attending Physician. FM or IM candidates will be considered Job duties and responsibilities: Attending at 1-2 facilities within WNY covering beds at Skilled Nursing Facilities Oversee supervision of 1-2 APPS EMR- Point Click Care On call for all facilities- 5p-8a 2-3 days per month Bill for 3 CPT codes based on level of acuity/involvement Work life balance- generally 8a-4pm with a great deal of flexibility Quality Bonus quarterly and a RVU productivity bonus available Typically, 17 visits per day expected. We recognize the importance of continuity of care and aims to ensure that we are here for you at each point in your recovery, including rehabilitation, long-term care and home health. As you transition from a hospital stay into a post-acute rehabilitation center, or to your home, our clinical team will work collaboratively with your primary care providers. They will serve your clinical needs, manage your care, and allow you to focus on what matters most your health.
