Key responsibilities of the RN Case Management Travel include:

• Perform comprehensive assessments of high-risk members.
• Collaborate with primary care providers to implement individualized care plans.

• Facilitate care transitions, coordinating with various healthcare professionals.

Requirements of the RN Case Management Travel include:

• Associates or Bachelor's Degree in Nursing or related field.
• Current, unrestricted state Registered Nurse license.
• Ability to analyze complex medical information and make decisions.
• Case Management certification.
• Experience with HH, OASIS, and HCHB.
