Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans.
- Provide therapeutic exercises, gait training, balance training, mobility improvement, and fall prevention education.
- Assess functional status and monitor patient progress toward rehabilitation goals.
- Complete accurate OASIS assessments and clinical documentation.
- Collaborate with physicians, nurses, and interdisciplinary team members to coordinate patient care.
- Document patient care using Epic Remote Client, Rover, and OASIS.
Requirements
- Active Physical Therapist (PT) license (Oregon or eligible; pending license accepted)
- Minimum 1 year of Skilled Home Health PT experience
- OASIS experience (required)
- Current BLS (BCLS) certification
- Valid driver's license, reliable personal vehicle, and auto insurance
- COVID-19 primary vaccination series plus one booster (required)
