Hospice Home Health Aide (HHA)

Overview

We are hiring a Home Health Aide (HHA) to work with hospice patients in a comfortable 6-bed Hospice Inpatient Unit. Because the unit has only 6 beds, your maximum caseload would be just 6 patients per shift.

Available Shifts
Evening Shift: 3:00 PM - 11:00 PM
Overnight Shift: 11:00 PM - 7:00 AM

Both positions include a rotating schedule with every other weekend required.

Responsibilities

• Follow the plan of care established by the primary nurse
• Report changes in patient condition to the appropriate nurse
• Maintain accurate patient care records
• Assist patients with personal care and hygiene
• Help patients perform prescribed exercises
• Provide a safe environment and report safety concerns
• Make beds and maintain patient rooms
• Prepare meals and assist with feeding as needed
• Perform light housekeeping tasks
• Provide respite for family caregivers
• Attend required in-service programs and team meetings

Requirements

• Current New York Home Health Aide (HHA) Certification required
• At least 2 years of experience as a CNA or HHA
• Authorized to work in the United States
