Hybrid Telehealth Patient Advocate
The Blue Venture Fund
Somatus is seeking a Patient Health Advocate (PHA) to support CKD and ESKD patients in a hybrid telehealth model. You will work closely with physician practices, spend time in offices, and serve as the primary Somatus representative to build trust and coordinate care. You will partner with clinicians to manage care plans, facilitate transitions, and ensure patient education and access to resources, including in-home visits and outreach to improve outcomes. #J-18808-Ljbffr The Blue Venture Fund
