The Licensed Practical Nurse (LPN) or Registered Nurse (RN), reporting directly to the Director of Nursing, is responsible for delivering, coordinating, and evaluating resident care in alignment with physician orders, individualized service plans, and regulatory standards. This role combines hands-on clinical responsibilitiesincluding medication administration, assessments, documentation, and direct resident supportwith leadership functions such as guiding care staff, promoting safety initiatives, and ensuring effective communication with residents, families, physicians, and community leadership. By integrating compassionate care with professional expertise, the LPN/RN supports the physical, emotional, and psychosocial well-being of each resident, while fostering a collaborative, resident-centered environment that upholds the community's standards of excellence.

Essential functions include providing services in accordance with nurse practice act and state/federal nursing center regulations, working with the Director of Nursing to provide direction and oversight to the care staff regarding the resident's needs, reviewing and revising assessments and care plans at admission, periodically per regulation, and at change in resident physical or psychosocial condition, and more.

Qualifications include graduation from an accredited nursing school and a current state license for LPN or RN is required at all times, 1+ years of nursing experience preferred but new graduates are welcome, CPR or BLS certification preferred, and demonstrated proficiency in common digital tools and applications.

Working conditions may need to walk or stand for extended periods and move throughout the community, resident apartments, and other areas of the community, while performing the duties of this job, the employee may occasionally be required to stoop, kneel, crouch, or crawl, as well as climb or balance, and this position may need to lift up to fifty (50) pounds independently, and up to 200 pounds with assistance.
