Roles & Responsibilities
- Evaluate and assess patients' physical therapy needs in a skilled nursing facility
- Develop and implement individualized treatment plans to improve mobility and function
- Provide therapeutic exercises and modalities to help patients reach their goals
- Collaborate with interdisciplinary team members to ensure comprehensive patient care
- Document patient progress and adjust treatments as needed
- Educate patients and families on therapeutic techniques and home exercises
Qualifications
- Current Physical Therapist (PT) license in the state of Oklahoma
- Previous experience in skilled nursing or rehab settings preferred
- Strong clinical assessment and treatment planning skills
- Excellent communication and interpersonal abilities
- Ability to work effectively as part of a multidisciplinary team
Knowledge & Skills
- Knowledge of physical therapy modalities, therapeutic exercises, and patient care techniques
- Understanding of Medicare/Medicaid regulations and documentation requirements
- Proficiency in electronic health records (EHR) systems
- Strong organizational and time-management skills
- Compassionate and patient-centered approach
