Job Description

**KEY RESPONSIBILITIES:**

**Patient & Family Support**

• Partner with Case Managers to guide patients and families through complex healthcare decisions and available resources
• Serve as a compassionate liaison between clinical teams and patients, translating complex information into actionable guidance

**Clinical & Program Support**

• Collaborate on clinical interventions that improve patient outcomes and quality of life
• Champion special medication fund programs and maintain current resource databases to ensure patients access critical support options
• Deliver comprehensive Medicare information and guidance to empower informed healthcare decisions

**Administrative Excellence**

• Organize departmental workflows and manage multiple priorities simultaneously while maintaining meticulous attention to detail
• Maintain confidential patient records and documentation with the highest standards of accuracy and professionalism
• Coordinate scheduling, correspondence, and communications to ensure seamless department operations

Qualifications

EDUCATION/EXPERIENCE REQUIRED:

High School diploma or G.E.D. equivalent. Associates degree, preferred or equivalent healthcare experience. Good verbal and written communication skills. Ability to read, write and speak English. Good critical thinking and problems solving skills necessary. Good organizational skills. Ability to multitask. Good interpersonal skills, including ability to develop and maintain trusting relationships. Ability to conduct self in a professional manner. Ability to be flexible. Ability to complete all assigned tasks with little supervision. Ability to move freely about the facility in order to visit patients.
