Position Summary:

The Clinical Program Specialist supports the development, implementation, and growth of innovative clinical and community-based healthcare initiatives across the GHN region. This role leads coordination and expansion efforts related to Enhanced Primary Care (EPC), Mobile Integrated Healthcare (MIH), and Perinatal Care Network (PCN) initiatives, with a focus on strengthening partnerships, supporting care transformation, and advancing sustainable models of care.

This position serves as a connector between health systems, providers, payors, EMS agencies, public health, community organizations, and regional partners to improve access to integrated, patient-centered care. The Clinical Program Specialist works collaboratively to identify emerging opportunities, support program development, facilitate stakeholder engagement, and advance projects that improve outcomes for priority populations.

Essential Job Functions

Program Development & Strategic Growth

  • Support the development, implementation, and growth of GHN clinical initiatives, including Enhanced Primary Care (EPC), Mobile Integrated Healthcare (MIH), and Perinatal Care Network (PCN) efforts.
  • Identify opportunities to expand clinical partnerships and recruit providers, organizations, and regional stakeholders into GHN-supported programs.
  • Assist in developing program strategies aligned with GHN priorities, regional health needs, and sustainable healthcare delivery models.
  • Collaborate with internal and external partners to support innovative care models that improve access, coordination, and outcomes.
  • Help identify and pursue new program opportunities that align with healthcare system transformation efforts.

Prenatal Care Network & Maternal Health Support

  • Support development of a regional Perinatal Care Network focused on improving access to maternal, child, and family health services.
  • Build relationships with perinatal providers, hospitals, doulas, community health workers, behavioral health providers, and community-based organizations.
  • Coordinate cross-sector collaboration to improve care pathways and referral systems for pregnant individuals and families.
  • Assist in identifying service gaps, strengthening provider partnerships, and promoting equitable access to care.
  • Support initiatives that address social determinants of health, care navigation, and family-centered service delivery.

Clinical Partnership & Stakeholder Engagement

  • Serve as a liaison between GHN and healthcare organizations, EMS agencies, clinics, hospitals, and community partners.
  • Develop and maintain strong relationships with providers, healthcare leaders, and partner organizations across the region.
  • Facilitate meetings, work groups, and shared-learning opportunities to support collaboration and alignment.
  • Engage partners in identifying challenges, barriers, and opportunities related to care delivery and system integration.
  • Promote regional coordination and alignment between healthcare and community-based services.

Practice Transformation & Systems Improvement

  • Support healthcare organizations in implementing patient-centered, integrated care models.
  • Assist providers and partners in advancing care coordination, behavioral health integration, and whole-person care approaches.
  • Collaborate with stakeholders to identify workflow improvements, care delivery enhancements, and partnership opportunities.
  • Help support system-level improvements that promote equitable, efficient, and sustainable community and clinical models of care.

Project Coordination & Program Support

  • Coordinate program activities, deliverables, timelines, and stakeholder communication.
  • Support planning and implementation of regional meetings, learning collaboratives, trainings, and events.
  • Track project milestones and support accountability for program objectives.
  • Maintain documentation, meeting materials, and reporting requirements.
  • Collaborate with internal GHN teams to ensure alignment across initiatives.

Data, Reporting & Quality Improvement

  • Support collection, tracking, and reporting of program metrics and outcomes.
  • Assist partners in using data to identify trends, improve workflows, and support decision-making.
  • Help monitor progress toward program goals and funding deliverables.
  • Participate in quality improvement efforts that support healthcare system innovation and measurable outcomes.

Workforce Development & Shared Learning

  • Support development of educational opportunities, webinars, and regional learning sessions.
  • Help share best practices, resources, and emerging models across partners.
  • Contribute to regional training initiatives that strengthen workforce knowledge and capacity.
  • Support planning for symposiums, convenings, and partner engagement opportunities.

Qualifications and Requirements

Education:


  • Bachelors degree in nursing (BSN)

Experience:

  • Minimum of three (3) years of experience in healthcare systems, clinical program coordination, population health, care transformation, public health, or related fields.
  • Experience working with interdisciplinary teams and community partnerships.

Preferred:

  • Masters degree in Nursing (MSN)
  • Experience working with perinatal care, maternal health initiatives, or family-centered services.
  • Familiarity with Mobile Integrated Healthcare (MIH), Community Paramedicine, or primary care transformation models, or Medicaid Transformation Project (MTP).
  • Experience supporting quality improvement, healthcare innovation, or value-based care initiatives.
  • Knowledge of Washington State healthcare systems, Medicaid programs, and community health partnerships.
  • Experience facilitating cross-sector collaboration among healthcare, public health, and community organizations .

Knowledge, Skills and Abilities:

  • Demonstrated ability to manage multiple projects and priorities simultaneously.
  • Strong relationship-building, facilitation, and communication skills.
  • Ability to travel regionally throughout South Central and Eastern Washington.
  • Proficiency in Microsoft Office Suite and virtual collaboration tools.
  • Knowledge of healthcare systems, and experience working across multidisciplinary environments to support innovation and care delivery transformation.
