Benefits Bi‑weekly pay Vacation, sick, and personal time Four medical plans to choose from, eligible first day of hire Dental Vision Aflac Disability and life insurance provided by the Agency, optional buy‑ups 401(a) Money Purchase Pension Plan, 401(a) Defined Contribution, and 457 Deferred Compensation retirement plans YMCA corporate discount Description (Summary) Provides case management support services and therapy services to families and youth determined to have a chronic Serious Emotional Disturbance (SED) or an Intellectual Developmental Disability (IDD) which impedes adequate functioning in one or more of the following areas: school, work, community, family, interpersonal relationships. Provides therapy to parents and family members as necessary to improve child functioning. Also responsible to provide outpatient therapy services to Youth as assigned. Responsible to the Program Supervisor for delivery of direct and indirect service units consistent with qualitative and quantitative performance goals and standards established for the department. Duties and Competencies Assesses the effects of the family on the condition of the child. Assesses the effects of the child upon the family. Assesses all members of the family unit. Understands and appropriately uses knowledge of basic growth and development and learning issues of children. Assesses and teaches parenting skills. Assess functioning in areas of school, community, family and interpersonal relationships. Assess growth and development of the child. Understands and appropriately uses strengths of the child and family. Understands and appropriately uses natural supports within and outside of the family unit. Coordinate development of a Family Plan of Service (FPOS) for all registered SED‑Child consumers of the Authority assigned to the caseload, including: Complete the Person Centered Plan (PCP) and other assessments for each case authorized to case management services according to approved procedures and formats required by Authority procedures and funding sources. Where indicated, maintain home based contact levels sufficient to assist the family in identifying its needs for family support and for active participation in the treatment planning and implementation process. Monitor implementation of the Family Plan of Service for each assigned consumer, including: Provide follow‑up and monitoring services related to FPOS goals and objectives and planned interventions, at indicated frequency levels. Complete periodic FPOS reviews and required assessments and status reports. Peer reviews are coordinated through and integrated with this process. Convene FPOS (including family participants) meetings if monthly and periodic reviews indicates that a significant revision of FPOS is warranted to continue or terminate service delivery. Coordinate services between all involved Authority personnel as defined by the established Family Plan of Service. Provide case management authorized services at quality levels that meet or exceed indicator thresholds established through the Continuous Quality Improvement Program as defined in the IPOS. Services may include: Assist consumer in procurement of supportive services, financial and medical benefits, and legal and habilitation services as needed. Provide individual and/or family therapy as needed. Provide consultative services to CMH staff people or other Authority personnel in order to enhance consumer functioning. Assist family members with problems relating to consumer conditions, needs and encourages their support in the treatment process. Consult with the psychiatrist around medication concerns and treatment plans. Assist consumers in obtaining hospital admission when therapeutically indicated. Provide, secure and/or advocate for necessary support services, including respite, as necessary to maintain and enhance consumer and family functioning in the community. Provide or procure daytime crisis intervention services as necessary. Coordinate referrals and services with other community agencies, implementing the Authority’s collaboration model. Implement interdisciplinary peer review recommendations and decisions on a retrospective and prospective basis. Maintain complete and detailed case records and service activity documentation consistent with DCH standards, Authority procedures and requirements of third‑party payor sources assuring revenue generation at levels which meet or exceed service targets. Enhance community understanding and acceptance of SED‑children as well as existing services through community presentations to school, civic and community agencies as assigned by Program Supervisor. Other related duties as assigned: Maintain individual credentials and participate in education and training as necessary to maintain current competency(s) in skill areas essential for implementing job duties and responsibilities. Minimal annual training hours is 24, with all of these specific to consumer population served. Makes presentations to the public including school, civic and community agencies and relevant meetings, as needed. Maintain current skills and developing new skills to maintain current competence essential for implementing job duties and responsibilities. Follows oral and written direction completing tasks as assigned by due dates. Corporate Compliance Responsibilities Complies with and maintains competency with all applicable laws, regulations, standards, and rules governed under Medicare, Medicaid, the Michigan Department of Community Health (MDCH), Michigan Department of Human Services (MDHS), Joint Commission, and any applicable funding sources. Understands, adheres and maintains competency with all MCMHA and CMHPSM standards, policies, procedures as they appear in MCMHA’s, Management of Information, Finance‑Contract Management, Recipient Rights, Performance Improvement, Administration, Human Resources, Treatment Foster Care, Environment and Provision of Care Policies. Complies with and maintains competency with the Health Insurance Portability and Accountability Act (HIPAA) regulations as they pertain to privacy and confidentiality. Interacts appropriately with Managers and peers. Performs required duties with an awareness of impact upon the Authority structure as a whole. Works as a Team Player within the entire Authority Structure. Qualifications – Education, Training and Licensing Credentials Master’s degree in social work, psychology or related clinical field with current Michigan licensure or certification. Requires a minimum of one year of experience in the examination, evaluation, and treatment of minors and their families. Experience with short‑term outpatient therapy techniques is preferred. Demonstrated knowledge and skills necessary to provide the appropriate care for the specific clinical needs and ages of the individuals served. Demonstrated knowledge of the clinical needs, symptoms, and behaviors of individuals served, as well as understand the principles of growth and development over the life span; have the ability to assess data reflecting each individual’s status; interpret the appropriate information needed to identify each individual’s requirements in relation to his or her age‑specific/population specific needs; and to provide the care needed by the appropriate age group (child, adolescent and adult). Other Competencies and Abilities Ability to work in a complex, detail‑oriented, deadline structured busy organization. Physical and mental abilities: frequent lifting or carrying up to 15 pounds, must be able to hear, frequent standing/walking up and down stairs due to community outreach basis, ability to communicate effectively in person and/or phone, the ability to read, analyze and interpret information, ability to write reports, correspondence and procedures, ability to maintain consumer charts/records, ability to define and solve practical problems, collect data, establish facts and draw valid conclusions, ability to influence others, and the ability to conduct patient assessments and treatment. Demonstrated proficiency in MS Outlook, MS Office, electronic medical records (Encompass II) and such other applications if/when required. Ability to be trained and successfully complete other computer training as required. Valid driver’s license, proof of current automobile insurance and driving record determined by Authority’s insurance carrier to be eligible for coverage if required as part of position’s duties. #J-18808-Ljbffr
