Responsibilities

• Administer medications and treatments per physician orders and center policy
• Monitor residents’ health status, record vital signs, and report changes to the supervising clinician
• Assist with activities of daily living and mobility, while maintaining safety and comfort
• Document care provided in the patient record accurately and timely
• Assist with admissions, transfers, and discharges following established procedures
• Collaborate with the licensed nursing team and other care staff to implement care plans

Requirements

• Active LPN license to practice
• Ability to work a guaranteed 40 hours per week in a travel assignment
• Experience in or willingness to work in a long-term care setting
• Strong communication and teamwork skills
• Must pass onboarding and health screening requirements
