As a triage RN, you'll be responsible for answering calls from hospice patients and families, assessing their needs, providing guidance, and coordinating care over the phone. You'll schedule home visits if needed and coordinate care with the care team, facility, and the patient's caregiver. Multistate license as a registered nurse required.Experience: Minimum three years of experience as a registered nurse in hospice required.Required: Reliable transportation.
