Hospice Registered Nurse Case Manager (RN) - Topeka, KS We are seeking a compassionate and experienced Hospice Registered Nurse Case Manager (RN) to join our team at Ascend Hospice in Topeka, KS. Hours: 8:30 am - 5:00 PM Sign On Bonus $5000 About the Role: As a Hospice Registered Nurse Case Manager (RN), you will be responsible for assisting the team with on-call, case management, and visits to ensure adequate coverage and call-outs. Completing initial assessments of patients and families to determine hospice care needs Regularly re-evaluating patients' nursing needs Using assessment data to determine care plan needs Collaborating with the healthcare team, patient, and family to direct the work of the team to provide continuous and well-coordinated care Counseling patients and families in meeting end-of-life needs Initiating appropriate preventative and palliative nursing procedures Communicating with physicians regarding patients' needs and reporting any changes in patients' conditions Supervising Hospice Aides Requirements: Associate's Degree in Nursing Must have a State Certified RN license 2 years' experience as a professional nurse, hospice experience preferred Must have a valid Driver's license and reliable transportation Must be willing to travel Strong verbal and written communication skills What We Offer: Competitive salaries Comprehensive healthcare benefits 401(k) retirement plan Paid time off Mileage reimbursement Opportunities to advance and grow your career And more!
