Registered Nurse - Case Manager

SSM-SLUH, Inc

Job Summary Assesses, develops, plans, implements, and evaluates patients’ discharge planning needs to the next level of care in collaboration with an interdisciplinary team. Primary Responsibilities Ensures compliance with Condition of Participation (CoP) discharge planning. Identifies and escalates gaps or barriers related to inpatient and observation length of stay. Assesses patients at high risk for readmission and identifies root causes. Develops plans and collaborates with a multidisciplinary team regarding discharge planning to the next level of care. Participates with other team members regarding opportunities for improvement in standard work. Other Responsibilities Utilization management process, ED case management process, and admission process. Acts as a case manager liaison with post‑acute providers (skilled nursing facilities, rehab, home health, etc.). Applies evidence‑based practice and scientific knowledge to nursing care based on patients’ age‑specific and clinical needs. Uses the ANA Code of Ethics for Nurses to guide responses to evolving health and nursing needs. Works in a constant state of alertness and safety. Performs other duties as assigned. Requirements Education: Graduate of an accredited school of nursing or equivalent education for licensing. Experience: Three years of acute care experience. Physical Requirements: Frequent lifting/carrying and pushing/pulling objects weighing 0‑25 lbs.; frequent sitting, standing, walking, reaching, and repetitive foot/leg and hand/arm movements; frequent use of vision and depth perception; frequent use of hearing and speech; frequent keyboard use/data entry; occasional bending, stooping, kneeling, squatting, twisting, and gripping; occasional lifting/carrying and pushing/pulling objects weighing 25‑50 lbs.; rare climbing. Licenses and Certifications: Registered Professional Nurse (RN) – Illinois Department of Financial and Professional Regulation (IDFPR). Registered Nurse (RN) – Missouri Division of Professional Registration. Registered Nurse (RN) – Oklahoma Board of Nursing (OBN). Registered Nurse (RN) – Wisconsin Department of Safety and Professional Services. Work Details Schedule: Day Shift (United States of America) Job Type: Employee Scheduled Weekly Hours: 40 Benefits Paid parental leave for newborns or newly adopted children (one week, pro‑rated based on FTE).
