Registered Nurse (RN) - Hospice

Hospice RN – Hospice
Shift: 8-hour days - Weekends: 1/month
Guaranteed Hours: May vary, facility may call off up to one shift per pay period (biweekly)
Minimum 1 year of skilled hospice experience
Experience with Home Care Home Base (HCHB) required
Current RN license in the state or compact license
BLS certification required
Provide nursing care and support to hospice patients in their homes.
Conduct pain and symptom assessments and manage medication needs.
Educate and support patients and families regarding end-of-life care.
Monitor patient conditions and report any changes to the interdisciplinary team.
Perform intake assessments and pressure ulcer staging/prevention.
Familiarity with Medicare and state hospice regulations is essential.
